In the area marked “Respondent,” enter the name of the place of public accommodation that you are filing against, its complete address (including county), and its telephone number.

Place an “X” next to the basis of discrimination (e.g., race, sex, etc.) that you believe occurred to you. Note: For Higher Education cases, this can only be “disability.”

Place an “X” next to the type of harm.

| Step 6 |
Write a brief description of what happened.
In your description of the facts, be sure to include the following information:
- State the name of the person(s) involved
- If reasons were given to you for the action, state each reason
- State all facts that show illegal discrimination was the real reason for the action. This can be comments, documents, statistics, people treated better than you, or any other facts.
- If you are alleging disability discrimination, be sure to identify your condition, and explain how it limits your abilities at home, at work, or any other way it restricts you in your daily life. Give examples.

Please print off the charge and take it to a notary. Do not sign the charge until you are told to do so by the notary. After you and the notary have signed the charge, please mail the signed charge to your local OCRC office.
This is not an official charge until it has been received and processed by the OCRC office.
The charge MUST BE NOTARIZED before it can be processed.
