The Ohio Civil Rights Commission takes our mission to investigate charges of discrimination and harassment very seriously. We also take seriously our responsibility as a state agency to protect the safety and security of all Ohioans.

In order to prevent and contain the spread of the COVID-19 virus, the Ohio Civil Rights Commission is suspending in-person charge filing at all of our regional offices. We encourage all Ohioans who wish to file a charge to file online or call their regional office.

Charge of Discrimination Form

These Charge of Discrimination affidavits may be printed, filled out in their entirety, signed under oath and mailed to one of our regional office locations.  They may also be printed and brought to a regional office location during open hours and notarized free of charge; however, due to public health precautions all regional offices are currently closed to the public.
