The Ohio Civil Rights Commission takes our mission to investigate charges of discrimination and harassment very seriously. We also take seriously our responsibility as a state agency to protect the safety and security of all Ohioans.

In order to prevent and contain the spread of the COVID-19 virus, the Ohio Civil Rights Commission is suspending in-person charge filing at all of our regional offices. We encourage all Ohioans who wish to file a charge to file online or call their regional office.

These Charge of Discrimination affidavits may be printed, filled out in their entirety, signed in the presence of a notary and mailed to one of our regional office locations.  They may also be printed and brought to a regional office location and notarized free of charge.
